3. כתב ערעור
הערעור יוגש בכתב תוך 60 יום מיום המצאת ההחלטה למערער והוא יפרט את הדברים הבאים:
(1)שמו, משלח־ידו ומקום מגוריו של המערער;
(2)שם החברה המנהלת את הבורסה שנתנה את ההחלטה שעליה מערערים ומען משרדה הרשום;
(3)מען בתחום שיפוטו של בית המשפט של ערעור לשם המצאה למערער;
(4)תאריך שבו קיבל המערער עותק מן ההחלטה אם נתקבל, או ציון שעותק כאמור לא נתקבל;
(5)נושא ההחלטה שעליה מערערים;
(6)נימוקי הערעור.
