8. הסמכת רופא וטרינר מוסמך
(א)רופא וטרינר שמבקש הסמכה להיות רופא וטרינר מוסמך, יגיש למנהל בקשה בכתב לפי טופס 5 שבתוספת הראשונה.
(ב)המנהל יסמיך רופא וטרינר לרופא וטרינר מוסמך אם ברשות המבקש –
(1)קורא שבבים;
(2)אמצעים ממוחשבים למסירת הדיווח וכתובת דואר אלקטרוני; ואולם רשאי המנהל, מטעמים מיוחדים שיירשמו, להסמיך לתקופה, גם רופא וטרינר שאין בידיו אמצעים אלה.
