3ב. כתב תשובה – כותרת
תיקון: תשפ״א
כתב תשובה יכלול בחלקו הראשון את הפרטים האלה בלבד ולפי הסדר המפורט להלן:
(1)הערכאה השיפוטית שאליה הוגש כתב הערעור וכן מספר התיק בבית המשפט;
(2)שם המערער ועורך דינו ופרטיהם כפי שהם מופיעים בכתב הערעור, ופרטיו של המשיב;
(3)שם פרקליטות המחוז;
(4)המען ופרטי ההתקשרות של המשיב ושל פרקליטות המחוז;
(5)המועד האחרון להגשת כתב התשובה.
