3. הודעה על מען
רופא יודיע בכתב על מען מגוריו ומען חדר עבודתו, ועל כל שינוי בהם, למנהל באמצעות מנהל המחלקה ולרופא המחוזי, תוך ארבעה־עשר יום מיום שניתן לו הרשיון או מיום השינוי, לפי הענין.
