4. שימוש בשם והתכתבות
בעיסוקו המקצועי ובהתכתבו עם משרד הבריאות ישתמש רופא בשמו הרשום ברשיונו; בכל התכתבות עם משרד הבריאות יציין הרופא בנוסף לשמו כאמור גם את מספר רשיונו.
