תיקון: תשע״ז
אבד רשיון של רופא, יודיע על כך מיד לתחנת המשטרה הקרובה ולמנהל; ההודעה למנהל תינתן בכתב באמצעות הרופא המחוזי ויפורטו בה נסיבות אבדן הרשיון, ויצורפו לה תצהיר לפי סעיף 15 לפקודת הראיות [נוסח חדש], תשל״א–1971, המאמת את העובדות המפורטות בהודעה, אישור בכתב מתחנת המשטרה על מסירת הודעה כאמור, ואישור על תשלום האגרה שלפי תקנה 8ב.
