7. אבדן רשיון
(א)אבד רשיון של רופא וטרינר, יודיע על כך מיד לתחנת המשטרה הקרובה ולמנהל; ההודעה למנהל תינתן בכתב ויפורט בה נסיבות אבדן הרשיון, ויצורפו לה תצהיר לפי סעיף 15 לפקודת הראיות [נוסח חדש], התשל״א–1971, המאמת את העובדות המפורטות בהודעה, ואישור בכתב מתחנת המשטרה שלה נמסרה הודעה כאמור.
(ב)ניתנה למנהל הודעה כאמור בתקנת משנה (א), רשאי הוא להחליף את הרשיון האבוד ברשיון חדש.
