6. הגשת בקשה לשינוי ברישום קיים
(א)בקשה לשינוי ברישום בפנקס תוגש בפורמט אלקטרוני שיפרסם המנהל ותכלול את הפרטים והמסמכים המפורטים בתקנה 3(א), בשינויים המחויבים.
(ב)על החלטת המנהל בבקשה לשינוי ברישום בפנקס יחולו הוראות תקנה 5(א) עד (ג).
