21. ניהול יומן מרשמים
(א)טכנאי שיניים מוסמך, טכנאי שיניים אומן וטכנאי שיניים מומחה ינהלו יומן מרשמים ובו יירשמו הפרטים שלהלן לגבי כל מרשם שקיבלו ממורשה לריפוי שיניים:
(1)תאריך המרשם;
(2)שמו ומענו של המורשה לריפוי שיניים;
(3)תיאור העבודה;
(4)תאריך מסירת העבודה.
(ב)טכנאי שיניים אומן או מומחה המעסיק טכנאי שיניים כאמור בתקנת משנה (א), ינהל יומן מרשמים מרכזי שלו ושל כל המועסקים אצלו.
