22. הודעה על מען
טכנאי שיניים יודיע למנהל בכתב על מען מגוריו ומקום עבודתו, ועל כל שינוי בהם באמצעות הרופא המחוזי, תוך שלושים ימים מיום שניתן לו הרשיון או מיום השינוי, לפי הענין.
