195. הודעה בדבר פניה לרשם
בקשה לרשם להכריע בשאלה אם אמצאה פלונית היא אמצאת שירות תוגש לרשם בכתב בצירוף האגרה שנקבעה ויפורטו בה העובדות החשובות להכרעה בשאלה וראיות לביסוסן, וכן יצויין בה מי הוא המשיב.
