9. תעודת חיסון תקפה
לצורך הוראות חלק זה תקפה תעודת חיסון בין־לאומית, במשך ששה חדשים שתחילתם מיום הששי למתן הזריקה הראשונה או במקרה של זריקה חוזרת, תוך ששה חדשים מן הזריקה הראשונה – משך ששה חדשים מיום בו ניתנה הזריקה החוזרת, אם נתמלאו בה תנאים אלה:
(1)מלאו אותה על כל פרטיה ולא נעשו בה תיקונים או מחיקות;
(2)אושרה בחותמת שלטונות הבריאות בארץ נתינתה.
