4. צירוף פסק דין
הואשם הנאשם בעבירת משמעת שהיא הרשעה בעבירה שיש עמה קלון שנעברה במילוי תפקידו כימאי או בקשר לכך, יצורף לתובענה העתק מאושר מפסק הדין הסופי שבו הורשע כאמור.
