8. הודעה על שינוי שם ומען
תיקון: תשל״ו־2
(א)חל שינוי בשם היצרן או במען מפעלו הנמצא בישראל, אחרי מתן הרשיון, יודיע על כך בכתב למנהל תוך שלושים יום מיום השינוי.
(ב)חל שינוי בשם היבואן או במען עסקו אחרי מתן הרשיון, יודיע על כך בכתב למנהל תוך שלושים יום מיום השינוי.
(ג)חל שינוי בשם היצרן או במען מפעלו הנמצא מחוץ לישראל, יודיע היבואן על כך בכתב למנהל תוך שלושים יום מהיום שבו נודע לו על השינוי.
