8. פרטי הרישום בתעודת הנכה
תיקון: תשס״ח, תשע״ה
בתעודת הנכה יירשמו פרטים אלה:
(1)שם משפחה ושם פרטי של הנכה;
(2)מספר הזהות של הנכה;
(3)מספרה הסידורי של התביעה שהגיש הנכה להכרה לפי החוק;
(4)תאריך תוקף התעודה;
(5)תמונת הנכה;
(6)סמל מדינת ישראל;
(7)סמל הנכים כמתואר בתוספת לתקנות הנכים (סמל הנכים), התשי״ג–1953.
