13. פרטים להשלמת ההודעה
המוסד רשאי לדרוש מהמבוטח שנפגע או ממעבידו פרטים נוספים ומשלימים בקשר לפגיעה בעבודה ותוצאותיה וכן בקשר לכל ענין אחר הקשור במבוטח שנפגע, העשויים לסייע למוסד בקביעת הגימלה לפי חלק ב׳ לחוק. הנדרש חייב למסור את הפרטים בכתב בחתימת ידו או בעל־פה בפני פקיד המוסד ולאשרם בחתימת ידו.
