מחלה ממחלות המפורטות בטור 1 לחלק ב׳ לתוספת השניה היא מתאריך שצויין בטור 3 מחלת מקצוע לגבי מבוטחים המועבדים בעבודה, במקצוע או בתהליך ייצור כמפורט לצד אותה מחלה בטור 2.
