3ב. כתב תשובה – כותרת
תיקון: תשפ״א
כתב תשובה יכלול בחלקו הראשון את הפרטים האלה בלבד ולפי הסדר המפורט להלן:
(1)ועדת הערר שאליה הוגש כתב הערר וכן מספר התיק בבית המשפט;
(2)שם העורר ועורך דינו ופרטיהם כפי שהם מופיעים בכתב הערר ופרטיו של המשיב;
(3)שם פרקליטות המחוז;
(4)מען ופרטי התקשרות של המשיב ושל פרקליטות המחוז;
(5)המועד האחרון להגשת כתב התשובה.
