3. תעודת התייצבות לרישום
יוצא־צבא וכן אדם בגיל 17 שנה שהתייצב לרישום כאמור בסעיף 3 לחוק, יצוייד בתעודה המעידה על התייצבותו לרישום, ובה יפורטו, בין היתר, שם משפחתו ושמו הפרטי ומספר תעודת זהותו.
