2. פרטי הערעור
אלה הפרטים שיכיל כתב ערעור:
(1)שם המערער, מספר הזהות שלו, ובתאגיד – מספר הרישום שלו, הטלפון שלו והמען להמצאת הודעות ומסמכים, ואם הוא מיוצג – שם עורך דינו ומענו; בפסקה זו, ”מען“ – שאינו בשטח המפונה כהגדרתו בחוק.
(2)התאריך שבו נשלחה ההחלטה נושא הערעור למערער;
(3)פרטי הנימוקים שבעובדה ובחוק שעליהם מתבסס הערעור;
(4)הסעד המבוקש.
