5א. הסמכת רופא וטרינר מוסמך
תיקון: תשע״ח
(א)רופא וטרינר שמבקש הסמכה להיות רופא וטרינר מוסמך, יגיש למנהל בקשה בכתב.
(ב)המנהל יסמיך רופא וטרינר לרופא וטרינר מוסמך אם ברשות המבקש –
(1)קורא שבבים;
(2)אמצעים ממוחשבים למסירת דיווח לפי תקנה 5(ג) וכתובת דואר אלקטרוני.
