תיקון: תשע״ד־2, תשע״ז
(1)הטופסים הנזכרים בתקנות אלה הם הטופסים הכלולים בתוספת השניה לתקנות אלה וכן טופס הגשה אלקטרונית; והטופסים הללו ישמשו לכל המקרים שהטופסים יפים להם ואפשר לשנותם כאשר יורה הרושם כדי להתאימם למקרים אחרים.
(2)כל הבקשות לרושם תהיינה ערוכות בכתב.
