66. תצהיר
תיקון: תשע״ז
(1)תצהיר יהיה ערוך בגוף ראשון, מחולק לסעיפים, ומכיל רק עובדות שהמצהיר יכול להוכיחן מתוך ידיעתו הוא או, אם ציין את הנימוקים לכך, גם לפי מיטב אמונתו.
(2)בכל תצהיר יצוינו שמו, תוארו וכתובתו של המצהיר.
