הוראה של פסיכיאטר מחוזי על אשפוז כפוי לפי סעיפים 9(א) ו־(ב), 11(ד) ו־15(ה) לחוק, על החזרה לאשפוז לפי סעיף 33(2) לחוק וכן על הארכת אשפוז לפי סעיף 10(ב) לחוק, תיערך לפי טופס 2 בתוספת; עותקים ממנה יישלחו לפי סעיף 38 לחוק ליועץ המשפטי לממשלה וליושב ראש התורן של הועדה הפסיכיאטרית.
