בתוקף סמכותי לפי סעיפים 12(א) ו־57(א) לחוק הפסיכולוגים, תשל״ז–1977, אני מתקין תקנות אלה:
1. פרטי רישום
בפנקס הפסיכולוגים יירשמו הפרטים שלהלן של כל אחד מהזכאים לרישום כפסיכולוגים, וכל שינוי בהם:
(1)שם המשפחה, השם הפרטי והשמות הקודמים, בעברית ובלטינית;
(2)תאריך הלידה ומקומה;
(3)המין;
(4)מספר תעודת זהות;
(5)שם האב;
(6)המען;
(7)התאריך שבו נרשם בפנקס;
(8)מספר הרישום בפנקס.
2. תחילה
תחילתן של תקנות אלה ביום תחילתו של החוק.
3. השם
לתקנות אלה ייקרא ”תקנות הפסיכולוגים (פרטי רישום), תשל״ז–1977“.
