2. מסמכים שיגיש מבקש
(א)בקשת מבקש תיערך בכתב ותוגש למרכז.
(ב)מבקש, שאינו שופט ימלא שאלון שימסור לו המרכז ויצרפו לבקשה.
(ג)לבקשה יצורפו פרסומים שפרסם המבקש וכל חיבור או מסמך אחר אותו ראה לנכון להביא לפני ועדת הבחירה.
