בעל רישיון או מחזיק שליטה בבעל רישיון, הרוצה לרכוש אמצעי שליטה בבעל רישיון נוסף, יגיש לרשות בקשה לכך, בכתב, ערוכה לפי טופס 2 שבתוספת, בשינויים המחויבים.
