הרופא הוטרינרי שערך בדיקה לפי תקנה 4, ימסור למי שהביא גופת חזיר בר לבדיקה ושנמצא חפשי משערונית, תעודה וטרינרית בנוסח שבתוספת השלישית, המתירה להעביר את הבשר ליעד שיצויין בתעודה.
