24. טפסים ותקנות
תיקון: [תשי״ד], [תשט״ז] [ע/27, ע/34]
(א)חוות דעת, תעודת רופא ותעודת עובד הציבור ייערכו לפי הטפסים הקבועים בתוספת הראשונה או בדומה להם ככל האפשר.
(ב)שר המשפטים רשאי, בתקנות, לקבוע דרך הגשתן של חוות דעת, תעודת רופא או תעודת עובד הציבור ולהביא שינויים בטפסיהן.
