5. כתב הערר
(א)כתב הערר יוגש מודפס, חתום ביד העורר או בא־כוחו, למען כאמור בתקנה 3(א).
(ב)כתב הערר יוגש במספר עותקים כמספר החברים בבית הדין והמשיבים ובצירוף עותק נוסף.
