6. פרטי הערר
(א)אלה הפרטים שיכיל כתב ערר:
(1)שם העורר, מספר הזהות שלו, ובתאגיד – מספר הרישום שלו, מענו, מספר הטלפון שלו והמען להמצאת הודעות ומסמכים;
(2)שמו ומענו של המשיב;
(3)תמצית ההחלטה שעליה עוררים;
(4)התאריך שבו הומצאה ההחלטה נושא הערר לעורר;
(5)העובדות שעליהן מסתמך העורר;
(6)הנימוקים שעליהם מבסס העורר את עררו;
(7)רשימת מסמכים שעליהם מסתמך העורר, בציון המסמכים שאינם מצויים ברשותו;
(8)הסעד המבוקש.
