המבקש גריעת שיק מהמנין או ביטול הגבלה לפי תקנות אלה, ימציא לבנק, לפי דרישתו, תעודה מאת רשות צבאית המאשרת כי שירת שירות חירום בציון התקופה שבה שירת, או שחלות הנסיבות האמורות בתקנה 3.
