18. הסכמתו של אדם
הסכמתו של אדם כאמור בסעיף 24 לחוק תינתן לפי טופס 5 שבתוספת; הסכמתו של אפוטרופסו של אדם, כאמור בסעיף 24(ב) לחוק, תינתן לפי טופס 6 שבתוספת.
