3. בקשה לרישיון או לחידושו
(א)המבקש לקבל רישיון הפעלה או המבקש לחדש רישיון כאמור (בפרק זה – מבקש הרישיון) יגיש לממונה בקשה לכך.
(ב)בקשה לחידוש רישיון הפעלה תוגש לממונה 45 ימים לפחות לפני מועד פקיעת תוקפו של הרישיון.
(ג)השר יקבע בתקנות הוראות לעניין הגשת בקשה לפי סעיף זה, לרבות הפרטים שתכלול ומסמכים שיצורפו אליה.
